Papillary thyroid carcinoma originating from thyroglossal cyst duct (TDC) is an accidental finding that presents less than 1% of a thyroglossal cyst, these usually present as midline swellings without any clinical presentations or rise in parameters. A 28-year-old female came to the ENT outpatient unit with a history of slow-growing, and swelling over the left side of her chin and neck for 13 years, not associated with dysphagia, dyspnea, or change in voice. Clinical, radiological, and analytical controls are done for the management of papillary carcinoma in the TDC. The outcome of the papillary carcinoma does not seem to be significantly impacted by routinely performed thyroidectomy.
